Membership Fees
One-time $100 enrollment fee per member
along with a MINIMUM of 3 months due upon registration and this payment is non-refundable.

*Family plans will only require 1 month due at a time.
First payment is $175
this will cover the first 3 months and the enrollment fee.

We only collect monthly for families.

After the first quarterly payment, the Membership Fee will be billed monthly. Membership Fees shall not be prorated or refunded for any month. Patients may terminate this Membership Agreement at any time and for any reason, upon providing advance notice to the practice. Such termination shall be effective on the last day of the then current paid date of the month.
Monthly Membership Fees will continue to accrue until Patient’s notice of termination is received by Practice.
Re-Enrollment Fee is $100 +Your first 3 months
