Membership Fees
A one-time enrollment fee per member
along with a MINIMUM of 3 months are due upon registration. This payment is non-refundable.
*If you need to be seen urgently there will be an additional emergency fee due of $100. We do our best get urgent New Patient appointments in with in two weeks.​
*Family plans will only require 1 month due at a time.

Kids 0-18
$30 a month
*With an Adult MEMBERSHIP
$150 Enrollment fee is due before you can schedule the first appointment. The first 3 months of service ( $90) is due the day of the first appointment.

Adults 19-64
$70 a month
$150 Enrollment fee is due before you can schedule your first appointment. Your first 3 months of services ($210) is due the day of your first appointment.
adults 65+
$60 a month
$150 Enrollment fee is due before you can schedule your first appointment. Your first 3 months of services ($180) is due the day of your first appointment.
family of 3 or more
$145 a month
The $150 Enrollment Fee is due before any appointments can be scheduled. The max enrollment fee is ($600) for a new family.
We only collect monthly for families. Our family plan cannot be canceled for 6 months.
ENHANCED IMMUNE SUPPORT
$150 A MONTH
This plan requires a $500 Enrollment Fee that is due before you can schedule your first appointment.
The First 3 months of service ($450) is due the day of the first appointment.
After the first quarterly payment, the Membership Fee will be billed monthly unless the patient notifies us that we wish to do quarterly or yearly billing. Membership Fees shall not be prorated or refunded for any month. Patients may terminate Individual Membership Agreements after the first 3 months, and Family Plans after the first 6 months for any reason, upon providing advance notice to the practice. Such termination shall be effective on the last day of the then current paid date of the month.
Monthly Membership Fees will continue to accrue until Patient’s notice of termination is received by Practice.
* If you leave the practice and decided to come back there is a Re-Enrollment Fee of $150 +Your first 3 months of service at the current advertised rate. *
